This study is a collaboration between Ladybug House and Seattle University College of Nursing.

We are designing a freestanding, community-based palliative care home and hospice for children, adolescents, and young adults in Seattle called Ladybug House.

With expert skilled staff and comprehensive services, our mission is to offer a place for respite and renewal to individuals with life-limiting illnesses and their families and friends.

For this study the term "life-limiting illness" is defined as any diagnosis or illness that has the potential to dramatically reduce years of life lived or dramatically impact quality of life.
Examples might include: cancer, cystic fibrosis,
muscular dystrophy
, congenital heart conditions, epilepsy or cerebral palsy.

We plan to offer compassionate therapies which include pain management and support services for patients and their families that together incorporate the physical, psychological, social and spiritual needs of individuals.

We are interested in hearing about your experiences (as an individual who has cared for a severely ill child/adolescent youth/ young adult) and want your honest feelings, feedback and ideas.

If you agree to complete the questionnaire your answers are confidential. We appreciate your help.
